Hospice is a specialized concept of care designed to provide comfort and support to patients and their families when a life-limiting illness no longer responds to cure-oriented treatments. Hospice care neither hastens nor postpones death. The goal of hospice care is to improve the quality of a patient's life by offering comfort and dignity. A few weeks ago, our Spiritual Care Director, Gary O’Connor, and RN Case Manager, Heather Gallagher, had the pleasure of presenting the Town of Fallsburg Lion's Club on the benefits of hospice services. After an extremely educational presentation, individuals had the opportunity to share their hospice experiences or ask questions related to the care of their loved ones and community members.

Sharing with our Hospice Heart’s 💚 #trusthasaname #hospicecareI had a patient die on Saturday morning. This is not unusual, I am a hospice nurse and an end-of-life doula, death is something I experience almost daily. And while I always know the outcome, sometimes there is a patient who I am not ready to let go of yet. I try really hard not to get attached, but sometimes I do, and I know many of you who work in this field can appreciate what I am saying.
She was adorable. Her husband was adorable. And their kids were providing such loving care to both of them. She was so strong and never let anyone know just how much pain she was dealing with. I knew though... I saw it in her face. On Friday I spoke with her and her family on the phone twelve different times, helping them to treat her pain, getting new medication orders, and handing over all my tools to help them help her. I arranged for weekend visits, making sure they were going to be in good hands, and told them I would call them, and hopefully see them on Monday.
I wanted more time with her. (I know her family did too)
When I heard she died, I experienced so many different emotions. Part of me was worried that I didn't say or do the right things, wondering how I could have missed it. How did I not see this coming so fast? What could I have done differently? Part of me was also feeling sad... I felt this loss a little deeper than usual.
I cried. I cried a lot.
And after I was done beating myself up, I gave myself a pep talk, the one I give others in this field... Decline can happen slowly, it can go really fast, and it can also jump from zero to sixty in a matter of hours...sometimes even minutes. Our role is not to give an exact time, but to hand over the tools so the people who love them are prepared. I did that. I know I did that.
I have witnessed hundreds (and hundreds) of last breaths, but each time it feels like the first. I hope I never stop feeling this way. I always know what to do, and I know how it feels but I am reminded this is the first time that this particular person is dying, and I honor and respect that. And when that last breath is taken, whether I witness it or not, I always whisper, “goodbye, safe travels, you are loved.”
With each death I am reminded of how truly precious and fragile life is. I wish I could have seen her one more time, I wish I could have told her how lovely I thought she was… and while I didn’t have a chance to say goodbye, I know I did the very best I could for her and that is enough for me. I am filled with gratitude for the time I did have with her and that she and her family trusted me with her care.
On Saturday when I heard that she had died… I whispered my message to her… and then I called her family. They said, "Thank you for being here for us, when she died she was not in any pain, she was peaceful and she was finally able to sleep." And my heart smiled...
