Social Workers Lend Support to Hospice Care in the Hudson Valley
Tending to the emotional and mental needs of a loved one nearing the end of life is one of the most challenging aspects of becoming a caregiver. Hospice social workers assess a patient’s physical, emotional, and mental well-being, constantly evaluating the care plans’ effectiveness. Some matters they assist with include but are not limited to:

The assistance does not stop when a loved one passes away. Our hospice social workers care team act as liaisons connecting families to bereavement and counseling services and assisting with grief support.
