Hospice Care with No Interruptions

Hospice of Orange & Sullivan Counties, Inc. has worked for decades to redefine the meaning of at-home care. A common myth associated with hospice care is that it’s only administered in a patient’s family home. In actuality, hospice care is available wherever the patient resides, be it a household, our own Kaplan Family Hospice Residence, or even a nursing home. When care is provided in comfortable surroundings, the patient’s quality of life is enhanced, alleviating the family’s stress.

One goal of hospice care is to lessen the stressors on patients and families during a delicate time. When hospice care is provided under the same roof as daily care, patients, family members, and nursing home staff benefit.

Patients who receive hospice care within a nursing home can continue regular medical care, greatly reducing the need for agitating hospital visits, which might serve to worsen the patient’s condition. The care team at Hospice of Orange & Sullivan Counties, Inc. is dedicated to meeting the end-of-life needs of the patient in compliment to the required levels of care a nursing home must provide.

Our Hospice Aide Specialists are available within nursing facilities three to five days per week to make sure end-of-life care – and its benefits to patients and family – continues uninterrupted. Each hospice team includes an RN case manager to advocate for patient and family needs while collaborating with physicians regarding pain management and symptom control. Hospice of Orange & Sullivan Counties, Inc. also monitors and documents any medical changes in the patient and reports them to a hospice case manager. In addition, our nurses, social workers, and volunteers are available as needed to provide both patients and families with the comfort they deserve.
