Living Wills/Advance Directives
The best time to make decisions concerning your health care is while you can consider your wishes carefully and discuss them with your doctor, caregivers, family, and loved ones.
Five Wishes is a complete approach to discussing and documenting your care and comfort choices. It’s about connecting families, communicating with healthcare providers, and showing your community what it means to care for another. Five Wishes advanced directives is a legal document and an easy method to make your wishes known. Our shared experiences related to the coronavirus have reminded us of what is most important for many of us.
Health Care Proxy
The New York Health Care Proxy Law allows you to appoint someone you trust, such as a family member or friend, to make health care decisions for you if you lose the ability to make decisions yourself. By appointing a healthcare agent, you can ensure that healthcare providers follow your wishes. This person can also decide how your wishes apply as your medical condition changes.
Attached is an instructional guide for caregivers of hospice patients at home…hopefully enabling you to provide necessary care for your loved one.
